The lateral corticospinal tract (crossed / pyramidal) runs in the posterior part of the lateral funiculus. It carries voluntary motor commands from the motor cortex to alpha motor neurones in the anterior horn. Approximately 90% of corticospinal fibres cross at the medullary pyramids. Lesion causes contralateral UMN weakness (spasticity / hyperreflexia / positive Babinski).
