Correct answer: ACalcium sensitisation of troponin C increasing contractility without increasing intracellular calcium

Levosimendan sensitises cardiac troponin C to calcium increasing contractile force without increasing myocardial oxygen demand (unlike agents that increase intracellular calcium). It also opens ATP-sensitive potassium channels in vascular smooth muscle causing vasodilation. It has some PDE III inhibitory activity. It is used in acute decompensated heart failure.
