Symptomatic infant hypoglycaemia: IV dextrose 10% at 2 mL/kg (200 mg/kg). NEVER 50% dextrose in infants — hyperosmolar, causes venous sclerosis, rebound hypoglycaemia, cerebral oedema risk. Follow with 10% dextrose maintenance. IM glucagon fallback if no IV.
