A speaking valve (e.g., Passy-Muir valve) is a one-way valve that allows air in through the tracheostomy but directs exhaled air UPWARD through the vocal cords and out through the mouth/nose, enabling speech. The cuff MUST be deflated before placing a speaking valve – if the cuff remains inflated, exhaled air cannot escape (it is blocked from going through the valve and blocked from passing around the cuff), creating a completely closed system leading to air trapping, respiratory distress, barotrauma, and potentially death. This is a critical safety point.
