This relative hypocalcaemia increases neuromuscular excitability, causing paraesthesiae, carpopedal spasm, and potentially seizures. Total calcium is unchanged – only the ionised fraction decreases. Treatment: re-breathing into a paper bag (increases PaCO2), anxiolysis, and reassurance. The carpopedal spasm resolves as pH normalises.
