After the third shock in a VF/pulseless VT arrest, the Resuscitation Council UK 2021 ALS algorithm recommends: adrenaline 1 mg IV AND amiodarone 300 mg IV during the next 2-minute CPR cycle. Adrenaline is then repeated every 3-5 minutes (i.e., every other cycle). A second dose of amiodarone 150 mg can be given after the 5th shock if VF/pVT persists. Adrenaline improves coronary and cerebral perfusion pressure during CPR. Amiodarone stabilises the myocardial membrane and increases the likelihood of successful defibrillation.
