Autogenous bone (autograft) is the gold standard for periodontal regeneration because it uniquely combines all three essential biological properties of bone grafts: osteoconduction (provides scaffold), osteoinduction (promotes new bone formation signalling), and osteogenesis (contains viable osteogenic cells). The hematopoietic marrow within autogenous bone confers superior osteogenic potential compared to all alternatives. Allograft (D) and DFDBA (E) are allogeneic materials with reduced osteogenic capacity; xenografts (B) rely primarily on osteoconduction with limited osteoinduction; synthetic materials (C) are osteoconductive scaffolds alone. Although autogenous bone has drawbacks—donor site morbidity, limited harvest volume, and increased surgical time—these do not diminish its gold-standard status when available.
