Postural instability and gait disorder (PIGD) is the motor feature of PD that responds least to levodopa. It is thought to involve non-dopaminergic pathways (cholinergic, noradrenergic, serotonergic). This is in contrast to tremor, bradykinesia, and rigidity, which respond well to dopaminergic therapy.
