Conventional PD solutions have low pH (5.2-5.5) and contain high levels of glucose degradation products (GDPs) from heat sterilisation. GDPs (methylglyoxal, formaldehyde, 3-deoxyglucosone) are directly toxic to peritoneal mesothelial cells, promote fibrosis, neoangiogenesis, and accelerate membrane failure. Biocompatible solutions use multi-chamber bags (separating glucose from buffer until mixing before use), achieving neutral pH and very low GDP levels. The balANZ trial showed better preservation of residual renal function and peritoneal membrane integrity.
