The risk of nephrogenic systemic fibrosis (NSF) varies dramatically between gadolinium agents. Group II agents (macrocyclic: Gadobutrol [Gadovist], Gadoterate meglumine [Dotarem], Gadoteridol [ProHance]) have very high thermodynamic stability, extremely low NSF risk, and can be used in CKD G4-5 with appropriate clinical indication. Group I agents (linear: Gadodiamide [Omniscan], Gadopentetate [Magnevist]) have been associated with nearly all NSF cases and should be avoided in eGFR <30.
