Tramadol is partially renally excreted and its active metabolite (M1, O-desmethyltramadol) accumulates significantly in CKD/ESKD. This increases the risk of seizures (Tramadol lowers seizure threshold), serotonin syndrome (particularly if co-prescribed with SSRIs/SNRIs), and respiratory depression. Tramadol is poorly removed by haemodialysis. BNF recommends dose reduction and extended intervals in renal impairment. Alternative analgesics for neuropathic pain in dialysis include low-dose Gabapentin (after dialysis), Pregabalin (renally adjusted), or topical agents.
