BV in pregnancy is associated with increased risk of preterm birth, premature rupture of membranes, and low birth weight. BASHH and NICE recommend treatment of symptomatic BV in pregnancy. Oral Metronidazole 400 mg BD for 5–7 days is first-line (safe in pregnancy — extensive safety data). Intravaginal Metronidazole gel or Clindamycin cream are alternatives. High-dose single-dose Metronidazole (2 g) is avoided in pregnancy. Doxycycline is contraindicated. Fluconazole treats Candida, not BV.
