Loss of viral thymidine kinase produces cross-resistance to aciclovir and valaciclovir and usually famciclovir. Foscarnet directly inhibits viral DNA polymerase without activation by thymidine kinase and is preferred; renal function, electrolytes and infusion-related toxicity require close monitoring.
