True virological relapse after DAA therapy almost always occurs within the first 12 weeks (pre-SVR12) or by SVR24. Recurrence of viraemia >12 weeks after SVR12 is overwhelmingly due to reinfection rather than relapse. Phylogenetic analysis comparing pre-treatment and recurrent viral sequences can confirm whether this is a new strain (reinfection) or the original (relapse). Reinfection rates are significant in ongoing risk populations (PWID, MSM). Retreatment with DAAs is effective for reinfection.
