The risk of PML-IRIS (worsening inflammation as immune function recovers) must be anticipated — it occurs in approximately 20% and may require corticosteroids. Cidofovir, Cytarabine, and Mirtazapine have all been studied but none have demonstrated consistent benefit in clinical trials.
