Peripheral VA-ECMO returns blood retrogradely into the aorta and can markedly increase LV afterload. A failing LV may cease ejecting, causing distension, pulmonary oedema and stasis; active unloading with an Impella, vent, septostomy or selected IABP strategy should be considered.
