The pathophysiology involves: (1) acquired von Willebrand syndrome — continuous-flow pumps create high shear stress that cleaves large vWF multimers (essential for platelet adhesion), causing a bleeding diathesis; (2) GI AVM formation — loss of arterial pulsatility triggers angiodysplasia formation through a mechanism involving reduced intestinal mucosal perfusion and HIF-1α/VEGF-mediated angiogenesis (analogous to Heyde syndrome in aortic stenosis). The HeartMate 3 (centrifugal flow with artificial pulse) was designed to reduce this, and the MOMENTUM 3 trial showed lower pump thrombosis but GI bleeding rates remain significant. Management includes reducing anticoagulation intensity, octreotide, and endoscopic treatment.
