Per the 2020 ESC ACHD Guidelines and 2022 ESC PH Guidelines, bosentan has the strongest evidence base in Eisenmenger syndrome from the BREATHE-5 trial showing improved 6MWD and reduced PVR. PDE5 inhibitors (A) are also used but evidence is less robust. CCBs (C) are not recommended in Eisenmenger. Prostacyclins (D) are used in severe PAH but less studied here. Riociguat (E) is not studied in Eisenmenger syndrome.
