In CKD patients: (1) a single elevated troponin is NOT diagnostic of acute MI — many CKD stage 4-5 patients have baseline troponin above the 99th percentile; (2) the DYNAMIC rise-and-fall pattern is key: a >20% change from a known baseline (or serial values showing significant delta) suggests acute myocardial injury; (3) the 0h/1h algorithm can still be applied but with CKD-adjusted interpretation. The 2023 ESC ACS Guidelines recommend serial troponin measurements (not single values) in CKD. This patient's rise from baseline 55 to 500 ng/L (>20% change) with a clinical presentation consistent with ACS supports the MI diagnosis.
