Fibrinolytic agents used: tenecteplase (preferred — weight-adjusted single IV bolus), alteplase, or reteplase. Adjunctive therapy: aspirin, clopidogrel (not ticagrelor/prasugrel with fibrinolysis), and enoxaparin or UFH. Half-dose tenecteplase for patients ≥75 years.
