The TRILUMINATE Pivotal trial (2023) was the first RCT of transcatheter TV intervention: 350 patients with severe TR randomised to TriClip TEER + medical therapy vs medical therapy alone. Results at 1 year: (1) TR reduction: 87% of TEER patients achieved ≥1 grade TR reduction (vs 4.8% medical); (2) quality of life (KCCQ): significantly improved; (3) 6MWD: significantly improved; (4) composite hierarchical endpoint (death, TV surgery, HF hospitalisation, quality of life): significantly favoured TEER. This established TEER as the first evidence-based transcatheter TR intervention. The 2025 ESC VHD Guidelines position transcatheter TV TEER as Class IIb for symptomatic severe TR in patients at high surgical risk at experienced centres. Patient selection is critical — severe RV dysfunction may limit benefit. The CLASP TR and FORMA trials provide additional evidence for different device platforms.
