A previous NSTEMI is documented atherosclerotic cardiovascular disease, so this is secondary prevention and the patient is automatically in the ESC/EAS very-high cardiovascular risk category. For very-high-risk patients, the ESC/EAS LDL-cholesterol goal is both an absolute LDL-C below 1.4 mmol/L and at least a 50% reduction from baseline. His current LDL-C of 2.1 mmol/L is therefore above the ESC/EAS target. The <1.0 mmol/L target is only considered for selected patients with a second vascular event within 2 years despite maximally tolerated statin-based therapy, which is not described here.
