The 2024 ESC Hypertension Guidelines recommend a systolic BP target of 120-129 mmHg for most adults receiving pharmacotherapy (Class I). If not tolerated, the ALARA principle (as low as reasonably achievable) applies. This is more aggressive than the previous target of below 130 mmHg (B). Targets below 120 mmHg (D, E) are not recommended.
