The 2022 ESC/ERS PH Guidelines recommend IV epoprostenol for: high-risk PAH (Class I) and refractory PAH. Alternative prostacyclin pathways: SC treprostinil (avoids central line), inhaled iloprost/treprostinil (less potent).
