HF iron deficiency is present despite normal haemoglobin because ferritin is below 100 and transferrin saturation is low. Intravenous iron can improve functional status and may reduce rehospitalisation after acute HF. Blood loss and other causes require evaluation; oral iron generally has inadequate evidence for this HF outcome. Apply this conclusion only to the clinical circumstances stated, with appropriate specialist review, contraindication checks, shared decision-making and follow-up. Reassess if the physiology, treatment response or competing risk changes. The competing options would require a different haemodynamic profile, diagnosis, procedural indication or risk balance from the one described here.
