Correct answer: DWithdraw olmesartan and reassess the enteropathy

Olmesartan can cause a severe coeliac-like enteropathy with villous atrophy, often after prolonged exposure. Negative coeliac genetics makes coeliac disease very unlikely; drug withdrawal is both therapeutic and diagnostically informative before immunosuppression is considered.
