Asthma NG245: corrections to the iatroX summaries
Published · Sources checked
The iatroX UK guideline and UKMLA asthma summaries were updated on 8 November 2026 after checking the current primary sources.
NG245 was published on 27 November 2024. Its current recommendations include November 2025 inhaler-licensing notes.
Age-specific objective diagnosis
The summaries now distinguish the diagnostic sequence for people over 16 from children aged 5–16. They use the NG245 adult FeNO threshold of 50 ppb and child threshold of 35 ppb, and preserve the different bronchodilator-reversibility criteria.
Separate treatment pathways by age
The chronic-treatment summary distinguishes AIR and MART for people aged 12 and over, initial ICS plus SABA treatment for children aged 5–11, and monitored treatment trials in children under 5.
Exact inhaler licensing and escalation
The summaries now emphasise device, strength, age and regimen-specific licensing, together with the adherence checks, biomarker assessment, add-on trials and referral steps in NG245.
Acute asthma uses a separate emergency pathway
The acute section is explicitly an adult BTS/SIGN pathway. It replaces the previous suggestion to wait 24–48 hours before admitting a severe or deteriorating attack with urgent assessment and escalation.
