What you’ll work on
- Select empirical antibiotics for progressive diabetic foot infection and cardiorenal-protective therapy for albuminuric CKD.
- Recognise the presented tirzepatide eligibility exception and adjust basal insulin for overnight hypoglycaemia.
- Recognise unreliable HbA1c results and choose appropriate alternative glycaemic monitoring.
Think through a clinical case
Decide how you would approach this case before opening the question session.
A 44-year-old with type 2 diabetes has a chronic plantar foot ulcer. Despite 3 days of correctly taken oral flucloxacillin, the surrounding erythema has expanded to 4 cm. There is malodorous purulent discharge and devitalised tissue. There is no exposed bone, fluctuance, crepitus, limb ischaemia, fever, tachycardia or hypotension. They can take oral medication, have normal renal function, no penicillin allergy and no risk factors for MRSA or Pseudomonas infection. A deep tissue sample is taken and assessment by the diabetic foot service within 1 working day is arranged. Which is the most appropriate empirical antibiotic while awaiting microbiology results?
