ABIM (American Board of Internal Medicine) board certification is the milestone that confirms your competence as an internist. The Initial Certification exam is a one-day, 220-question computer-based test covering the full breadth of internal medicine. For fellows, subspecialty certification exams test specialty-specific knowledge.

The Resources
MKSAP — The Official Standard
MKSAP (Medical Knowledge Self-Assessment Program) is the official board review curriculum published by the American College of Physicians (ACP). It consists of a comprehensive textbook covering all 11 internal medicine subspecialties, accompanied by a Q-bank. Many residency programmes require or strongly recommend MKSAP.
MKSAP is not just an exam prep tool — it is the most authoritative review of internal medicine available. The textbook is dense and exhaustive. The Q-bank tests at the level of complexity expected on the ABIM exam.
The challenge with MKSAP is its volume. Completing the entire programme (textbook + Q-bank) takes 200–300+ hours. Starting early — ideally at the beginning of PGY-3 or the start of fellowship — is essential.
UWorld — The Q-Bank Standard
UWorld's Internal Medicine board review Q-bank is the highest-quality question practice available for ABIM preparation. The explanations are exceptionally thorough and teach clinical reasoning rather than just facts.
Many trainees use MKSAP as their textbook and UWorld as their Q-bank — reading MKSAP chapters and then practising corresponding UWorld questions. This combination is the most commonly cited approach among ABIM passers.
BoardVitals — The Affordable Alternative
BoardVitals offers an ABIM board review Q-bank at a lower price point than UWorld. Questions are written by board-certified physicians and include detailed explanations. BoardVitals is a credible alternative for trainees who cannot afford both MKSAP and UWorld.

6-Month Study Plan for Working Trainees
Months 1–3: MKSAP textbook reading (1 chapter per week). Do the corresponding MKSAP Q-bank questions after each chapter. Supplement with 15–20 iatroX adaptive questions daily for reinforcement.
