Your Hospital's Clinical AI and Your Own Reference: What to Keep Between Rotations

An access-continuity worksheet for doctors changing employer, undertaking locum work or moving to UK practice.

A doctor can keep the same name, email address and professional registration while three things change overnight: the employer paying for a reference service, the account permitted to open it and the clinical records that account may access. Moving hospital is therefore a useful time to check a clinical AI toolkit, especially when more of it sits inside an EHR.

AMBOSS's integration direction makes this issue concrete, but it applies across the market. An institutional assistant may be excellent during a placement. A reference and learning routine should also have an explicit plan for the gap between placements.

Published by iatroX. The product descriptions below were checked against public documentation on 26 October 2026. The worksheet is an editorial planning tool, not a promise about any employer's licence or individual account.

Separate login, entitlement and record permission

A login establishes an identity. An entitlement establishes which product features that identity can use. An employer's clinical-system permission establishes which records and workflows the doctor may access for their role. A successful sign-in does not settle the other two questions.

AMBOSS's published US terms link institutional use to authorisation and authentication, and describe access ending when the relevant institutional entitlement ends. The same terms distinguish individual licences. The practical question is which arrangement applies to your account and which components it covers.

Avoid a vague diary entry saying "renew medical apps." Write down the actual service and its access route. A library reached through the hospital network, an individually purchased question bank and an EHR-launched assistant can have different renewal dates even when they share one brand.

Make an entitlement sheet before leaving

Use the following worksheet while the current employer's library or digital team can still answer questions. Keep account-support information separate from patient information.

ItemQuestion to resolveUseful record
Account identityWill the registered email and recovery route remain available?Current recovery method and supplier contact
Reference entitlementWhich features end with employment or placement?Product components and confirmed end date
Personal continuationIs a suitable individual offer available for this account and country?Written scope, current terms and price link
Learning activityWhat personal learning material can be retained or exported, if supported?Supported method and content permissions
EHR accessWhen does the old role end, and who authorises the new one?Employer contact and local onboarding step
Local guidanceWhere is the new employer's approved policy source?Authorised portal and induction contact
Country and purposeDoes the intended use remain permitted in the destination?Applicable product terms and local policy

Treat unknowns as tasks with an owner and date. "Ask the library on Friday" is more useful than assuming the app will keep working because its icon remains on the phone. The aim is to find a gap early enough to choose a suitable replacement or continuation route.

Work through a fictional rotation change

Imagine a doctor moving from Hospital A to Hospital B, with a fortnight of exam study between contracts. At Hospital A, a clinical reference opens through the EHR. The doctor also uses an online question bank with a personal account.

Before leaving, the doctor confirms when Hospital A's institutional entitlement ends and asks whether the reference account can continue under an individual arrangement. They check the question bank's separate access period. They identify personal revision notes they are permitted to keep, rather than copying clinical queries wholesale.

During the fortnight, the relevant task is learning: revising concepts and inspecting general evidence. At Hospital B, the doctor completes local onboarding before using the new EHR assistant. They find the current local pathways instead of treating remembered instructions from Hospital A as transferable policy.

This example contains no product failure. It shows how a sensible toolkit can involve several permissions that change at different times. The same exercise helps a locum with multiple employers or an international doctor arriving before a first NHS post starts.

A country change needs a purpose check

A platform's availability in a browser does not establish that every feature has the same intended use in every country. Check the exact regional product, feature and applicable terms, particularly when moving between education, literature research and clinical use.

AMBOSS's US terms distinguish Clinical Care use in named countries from medical-literature-research use elsewhere and restrict personal-data input. Special institutional arrangements may differ. Ask which terms cover the actual service; do not infer enterprise permissions from a standalone account or from a demonstration.

iatroX also states a scope distinction: its clinical AI standards separate UK guidance-led clinical use from international educational and exam-facing use. For a doctor moving to the UK, this supports a deliberate transition from destination-specific preparation to checking UK sources in practice. It does not make every international question a UK clinical question.

Record the country and purpose next to the service on the entitlement sheet. Review them again when the task changes, rather than only when the subscription renews.

Compare institutional convenience fairly

EHR access has a strong practical case. The institution can provide a consistent route for its team, establish local responsibilities and configure the workflow. Where authorised patient context is available, clinicians may avoid repeatedly reconstructing the same information in a separate question.

The market contains different arrangements. UpToDate documents EHR links and contextual Infobutton integration. Cedars-Sinai describes enterprise OpenEvidence access using relevant patient-record information. Neither description tells a departing doctor that an employer's record permissions travel to the next job.

AMBOSS's roadmap separates currently available in-chart search from future chart-aware support. When reviewing access, name the actual deployed feature. "I have AMBOSS" is too broad to tell a new employer what task you expect it to perform.

An independent account can provide continuity for suitable reference or educational tasks. It also requires its own access, payment and intended-use checks. The strongest arrangement is one chosen around the doctor's tasks, rather than around a presumed winner between personal and institutional tools.

Keep the learning question without carrying the patient record

Professional continuity does not require taking a patient chart from one workplace to another. Keep patient information within the employer's authorised systems and follow the local process for access ending. A personal study record can instead capture the concept to revisit, the source to inspect and a future learning task.

For example, a neutral entry might read: "Review how study design changes the interpretation of a diagnostic test; attempt two relevant questions." It preserves an educational objective without retaining a clinical narrative. A new workplace can then supply its own patient context through its authorised workflow.

For the organisational question of changing software suppliers and moving records, see our separate article on healthcare AI data portability. The individual doctor's task here is maintaining lawful, appropriate access to knowledge and learning.

Give iatroX a defined place in the toolkit

For UK clinicians, iatroX offers a useful direct route for reference and continued study between jobs. Its published workflow distinguishes referenced lookup, structured Brainstorm reasoning and educational pathways. That makes it straightforward to assign a job to each surface instead of expecting one hospital integration to cover every professional need.

The learning follow-through is concrete. iatroX Tutor works from an attempted question, explores the learner's reasoning and supports creating cards for Anki export. A doctor preparing for the next examination can use that deliberate practice alongside whichever institutional reference the new employer provides.

Before the first shift, finish three checks: open the authorised local policy source, confirm the permitted clinical assistant and test the personal reference-and-learning route needed outside work. Keep the support contacts on the entitlement sheet. This gives the next rotation a practical starting point while allowing each service to do the job it is designed for.

FAQs

Will my AMBOSS account stop working when I leave a hospital?

The answer depends on the entitlement attached to the account. Confirm which institutional components end, whether another valid licence applies and what individual continuation options are available.

Does a personal reference subscription include access to a new employer's records?

A subscription and an employer's record permissions are separate. The new employer must authorise the clinical systems and workflows appropriate to the doctor's role.

Can iatroX replace the clinical AI supplied by my hospital?

Choose by task. iatroX can support suitable UK reference and educational work; an authorised chart-connected workflow needs separate evaluation. Many doctors will find a complementary arrangement useful.
