From ABEM Sample Videos to Unseen Cases: Test Whether Your Certifying Exam Preparation Transfers

An ABEM sample case is useful when you can identify the task it demonstrates and perform that task in a different situation. Remembering the order of the sample is a weaker test. Preparation should therefore include a deliberate step between watching official material and attempting an unseen case: explain what skill the example is asking you to show.

The ABEM Certifying Exam content page, checked on 13 September 2026, distinguishes Clinical Care Cases from Communication and Procedure Cases and provides sample and debrief resources. Use those current categories rather than assuming the certifying assessment is simply a spoken version of the written Qualifying Examination.

Watch for the decision, not the candidate's exact wording

When reviewing a sample, pause at a consequential point. Ask what information the candidate has, what decision is being made and what would change it. Then resume and compare the reasoning with the official explanation where one is provided.

Do not transcribe the encounter into a script to repeat. The exact sequence may depend on details that will not recur. A useful note is a conditional principle in your own words, together with the information that makes it applicable.

This article does not report an independent assessment of the sample videos or their educational effect. The method below is a proposed way to use the published material, with original exercises rather than reconstructed examination cases.

Build a variation around one dependency

In an original fictional exercise, a clinician is organising the next steps for several patients. One is waiting for additional information, another requires reassessment, and a new concern is reported about someone already seen. The learner must explain which information changes the priority and how they would communicate responsibility.

Do not make the exercise difficult by adding an arbitrary number of emergencies. First identify a single decision point. In the first version, the new concern is unverified and needs prompt clarification. In the second, the information is confirmed and materially changes the situation. The learner should show why their reasoning changes, not merely announce that everything is urgent.

The exercise deliberately contains no treatment instructions or official scoring. An appropriate clinical educator should determine the clinical detail and evaluate the response when using it for formal teaching. The independent learning task is to make the dependency between information and action explicit.

Change the audience as well as the facts

A learner may explain their reasoning clearly to an examiner but struggle to communicate the same issue to a patient or colleague. That is a useful variation, provided it is labelled honestly as a teaching exercise rather than an assumed ABEM station.

For a patient-facing version, ask the learner to explain why the plan has changed without overstating what is known. For a colleague-facing version, ask them to communicate the immediate responsibility and outstanding uncertainty. For an examiner-facing discussion, ask them to justify the decision and describe what would make them reconsider.

The information can remain related while the communication task changes. Successful transfer means recognising that difference, not delivering the same explanation with a different greeting.

An observation-to-practice worksheet

StageQuestionProduct of the exercise
ObserveWhat task does the official material demonstrate?A short description in your own words
ExplainWhich information supports the decision? The consequential evidence, not the full transcript
VaryWhat original change would test the same skill?A new detail with a reason for including it
AttemptCan I respond without consulting the original sequence? An uninterrupted fictional practice response
ReviewDid I adapt to the changed condition?A specific observation and correction
RevisitCan I repeat the reasoning in another context?A later, different practice task

This worksheet is an original educational asset. It is not an ABEM rubric, an estimate of examination performance or a licence to reproduce protected assessment material. Keep your notes focused on your own understanding.

Identify the kind of failure before buying more cases

A missed variation can expose several different needs. The learner may not understand the clinical principle, may overlook the changed detail, may be unable to organise a spoken response, or may address the wrong audience. Those problems should not all produce the same recommendation to complete more cases.

For a knowledge gap, use authoritative learning material and relevant questions. For failure to notice changing information, compare the two case versions and identify the ignored difference. For a communication problem, obtain feedback from someone who can describe what the listener understood.

Physical procedures require their own appropriate practical preparation. Describing a procedure or responding well in a conversational case does not establish that it was performed competently. Preserve ABEM's distinction between assessment tasks instead of allowing a convenient digital activity to stand for all of them.

Where iatroX can complement official preparation

This comparison is published by iatroX and includes its own learning resources. Official ABEM materials remain the authority on the assessment. The September 2026 iatroX simulation announcement lists an ABEM Certifying track, with text and voice practice, transcript-linked feedback and Tutor-led follow-up.

Use a relevant encounter to practise responding to unfamiliar information, then review the reasoning attached to the actual exchange. Do not assume that iatroX reproduces ABEM's assessment environment, uses its scoring process or predicts a pass. A written ABEM question session is another activity again: it can address knowledge without replacing the certifying tasks.

Per September 2026 iatroX product information, the UK price is £99 paid upfront annually, equivalent to £8.25 a month billed annually, or £29 monthly, covering paid banks, Tutor, study planning, simulations and CPD tools. These are UK advertised prices, not a US-dollar comparison; one complete simulation is free.

A practical verdict

Start with official samples when the assessment task is unclear. Use faculty or experienced clinician feedback when the quality of the reasoning or interaction needs expert scrutiny. Add varied cases when the learner understands the task but needs further attempts that are not already familiar.

Before resetting a case collection, examine whether the next attempt will test something new. A repeated case can still be useful for practising a correction, but label it as repeated practice. Its score should not be treated as equivalent to performance on an unfamiliar case, and scores from different products should not be combined into a single readiness estimate.

Frequently asked questions

Should I memorise the sequence shown in ABEM's sample videos?

Use the samples to understand tasks and expectations, then practise the underlying reasoning in original variations. A remembered sequence does not establish that you can adapt to new information.

Are the exercises here official ABEM cases?

No. They are original educational exercises intended to complement, not reproduce, official preparation material.

Can a written question bank replace Certifying Exam practice?

No. It may support knowledge, but the certifying assessment includes different applied and communication tasks that require relevant preparation.
