A four-week MRCEM OSCE plan should match the activity to the time available around emergency department shifts. Use short sessions to repair a specific knowledge or communication gap, longer protected periods for complete stations, and appropriate local teaching for practical skills. Do not expect a full mock circuit to fit reliably after every clinical shift.
The RCEM calendar, checked on 3 October 2026, lists the London MRCEM OSCE sitting for 3-19 November 2026. Work backwards from your own allocation, not the first day of that window. A candidate allocated 3 November can use 6 October to 2 November as a four-week frame; later allocations require a corresponding adjustment. See the official calendar.
Start with the station task and the actual rota
The RCEM MRCEM examination page, checked on 3 October 2026, describes an OSCE of 16 eight-minute stations with reading time and rest stations. Use the current candidate instructions when recreating conditions, and keep MRCEM requirements distinct from FRCEM material that may appear on the same website.
Mark work, travel and fixed responsibilities before adding practice. Then identify which periods can support spoken rehearsal, which can support a short source review and which should remain unallocated. A plan built around an ideal weekday routine may fail when shifts change.
This is a proposed preparation framework, not an RCEM-endorsed schedule or evidence that four weeks is sufficient for every candidate. The starting point and access to suitable observation matter.
Week one: observe what breaks down within a station
Begin with different station tasks rather than only familiar clinical presentations. Ask an observer to distinguish knowledge, practical performance and communication. A candidate may know an appropriate plan but fail to make the requested action clear to a colleague; another may communicate smoothly while omitting a necessary part of the task.
Read the candidate instruction carefully before practice. Identify the role, the intended recipient of the communication and the action required. A station that asks for a focused discussion should not automatically become a complete history because that is the opening sequence you have rehearsed most often.
Use the RCEM OSCE information and FAQs, checked on 3 October 2026, as the official reference for current preparation questions. Do not treat an unofficial checklist as a complete account of domain-based assessment.
Separate brief learning from full performance practice
A short session can clarify a clinical distinction, rehearse a concise explanation or inspect an unclear handover. It cannot demonstrate how the whole station unfolds under time pressure. Conversely, repeatedly running full stations may expose a weakness without providing the smaller exercise needed to correct it.
Choose the task before opening the resource. If the problem is unclear escalation, practise stating the situation, the relevant concern and the help required. If the problem is a practical technique, arrange supervised practice with appropriate equipment rather than trying to resolve it through dialogue alone.
RCEMLearning's MRCEM OSCE material, checked on 3 October 2026, provides a College-linked learning resource. Check access and the relevant material in advance so that a protected session is used for learning rather than account setup.
Week two: use a fictional handover to practise clarity
A fictional candidate, Isaac, receives feedback that his handovers contain relevant information but leave the colleague unsure what he is requesting. He responds by making them shorter, yet the observer still cannot identify the required next action.
Isaac's revised goal is not brevity alone. He identifies the purpose of the handover, selects the information needed for that purpose and states the request explicitly. The colleague then responds with a question or uncertainty, requiring Isaac to clarify rather than repeat the whole summary.
In a later variant, the colleague disagrees about the proposed next step. Isaac must explain his concern and respond appropriately within the fictional scenario. This exercise tests communication of clinical reasoning without prescribing management for a real patient or claiming that one verbal framework fits every station.
Week three: organise a small observed circuit
Use a group session to test transitions between different tasks and to expose weaknesses that do not appear in isolated rehearsal. Allocate candidate, role-player and observer functions clearly. The observer should record specific actions and omissions rather than providing only an overall impression.
Keep the feedback connected to the station's purpose. A polished history is not necessarily successful performance if the task was to communicate an agreed plan or demonstrate a practical skill. Ask what the candidate was expected to achieve and whether the interaction made that clear.
After the circuit, repeat selected tasks with the identified change. Do not simply collect scores and move to another circuit. An unfamiliar variation can show whether the correction transfers beyond the original scenario.
Place iatroX between, not instead of, observed sessions
In its September 2026 simulation launch description, iatroX includes an MRCEM OSCE track with voice and text practice, coaching, uninterrupted exam mode and transcript-linked feedback. A useful role is repeated conversational rehearsal between human-observed sessions, followed by focused review of an exchange that caused difficulty.
These features do not establish practical competence, official examiner endorsement or a validated pass prediction. A transcript cannot show whether a manoeuvre was performed correctly or capture every non-verbal aspect of teamwork. Keep bedside, procedural and equipment-based practice in the appropriate supervised setting.
According to iatroX's October 2026 product information, one complete simulation is free and ongoing access is included with question banks, Socratic Tutor, study planner and CPD tools in the paid bundle. Simulations are not a separate add-on. The planner's quiz-based learning support does not mean the platform manages your clinical rota or books group practice.
Week four: prioritise the recurring problems
Review the patterns that remained after the observed circuit. A knowledge uncertainty needs a source check and fresh application. A communication problem needs another interaction. A practical weakness needs appropriate teaching and observation. Do not choose a convenient digital task merely because the more relevant activity is harder to arrange.
Keep some complete station rehearsal, but stop expanding the resource collection without a specific reason. A late unfamiliar framework can add another thing to remember while leaving the original difficulty unchanged.
Confirm logistics and current instructions for your allocation. Ensure that planned practice conditions correspond to the relevant examination rather than an older course description. A preparation article is not a substitute for the information issued to you as a candidate.
Make the fallback plan explicit
When a shift overruns, decide which activity can reasonably move and which needs to be protected. An occasional missed solo case is different from losing the only practical teaching session or observed mock in the week. Preserve the work that supplies evidence you cannot obtain alone.
Leave a clear restart point after an interrupted session: the exact uncertainty and the next action. Avoid turning every interruption into a requirement to reread the whole topic.
The plan succeeds when its different activities address different preparation needs and remain possible within your week. More cases are useful only when they lead to clearer decisions, better-observed performance or a specific correction that can be tested again.
Frequently asked questions
How should I revise for the MRCEM OSCE after a long shift?
Choose a bounded task that fits the available attention and time, rather than automatically attempting a full circuit. Protect separate periods for observed and practical preparation.
Can AI simulation replace hands-on MRCEM OSCE practice?
No: it can support appropriate conversational rehearsal, but it cannot establish practical technique or all aspects of clinical performance. Arrange suitable supervised practice for those skills.
Should all November candidates start the same four-week plan?
No: the published sitting spans several dates, so work backwards from your own allocation. Confirm the current instructions and practical arrangements for that appointment.
