Prescribing considerations
- Confirm a susceptible or clinically appropriate bacterial indication and consider local antimicrobial policy.
- Do not rely on oral phenoxymethylpenicillin for severe acute infection or where vomiting, gastrointestinal disease or impaired absorption may prevent reliable exposure.
- Clarify the nature and severity of previous penicillin, cephalosporin or other beta-lactam reactions.
- Review renal function in marked impairment and consider combined hepatic and renal dysfunction.
- Consider potassium content in renal impairment, controlled-potassium diets or concurrent potassium-raising treatment.
Contraindications and cautions
- Known hypersensitivity to penicillins or any product excipient.
- Sugar-free sorbitol-containing oral solutions are unsuitable for hereditary fructose intolerance.
- The selected lactose-containing tablet is unsuitable for certain rare hereditary disorders affecting galactose or lactose handling.
Monitoring
- For prolonged treatment, consider full and differential blood counts and renal and hepatic function.
- Monitor INR where clinically indicated with warfarin or another coumarin anticoagulant.
- Assess persistent severe diarrhoea promptly for antibiotic-associated colitis.
- Review for resistant bacterial or fungal superinfection during prolonged use.
Clinical pharmacology
An acid-stable, orally active, beta-lactamase-sensitive penicillin with bactericidal activity. Absorption is rapid but incomplete and reduced by food. Elimination is mainly renal and may be prolonged in renal impairment.
Formulation and product differences
- The selected tablet contains lactose and is swallowed with water.
- Selected standard oral solutions contain sucrose, saccharin and flavourings.
- Selected sugar-free oral solutions contain sorbitol and strawberry flavouring; hereditary fructose intolerance is an important excipient restriction.
- Selected reconstituted liquids require refrigerated storage and have a limited in-use shelf life; sugar-free liquids should be shaken well before measuring.
- Excipients, preparation instructions and the recommended interval after food differ between products; check the individual label and leaflet.
