Prescribing considerations
- Confirm the licensed indication, age range and device suitability; beclometasone products are not automatically interchangeable.
- Assess cumulative corticosteroid exposure and adrenal-suppression risk, especially with prolonged treatment, transfer from systemic steroids or strong CYP3A inhibitors.
- Check inhaler technique, adherence and access to appropriate acute-relief treatment.
- Use topical products cautiously on the face, flexures, large areas, damaged skin or under occlusion; exclude or treat infection.
- Before Clipper, assess infection, hepatic or renal impairment, diabetes, peptic ulceration, hypertension, osteoporosis, adrenal impairment, glaucoma, cataract and psychiatric history.
Contraindications and cautions
- Hypersensitivity to beclometasone dipropionate or product excipients.
- The selected topical products are contraindicated for primarily infected fungal, bacterial or yeast skin lesions and dermatoses in children under 1 year. They are unsuitable for acne, rosacea, perioral dermatitis and untreated perianal or genital pruritus.
- Clipper is contraindicated in tuberculous, local fungal and viral infections; severe hepatic impairment is not recommended because data are absent.
Monitoring
- Review asthma or rhinitis control, adherence and administration technique.
- Monitor growth in children receiving prolonged inhaled or intranasal corticosteroids.
- Consider adrenal assessment with substantial cumulative exposure or symptoms of adrenal suppression.
- Investigate blurred vision or other visual disturbance.
- Monitor treated skin for infection, atrophy, striae, pigment change and withdrawal reactions.
- With Clipper, monitor disease response, infection, psychiatric symptoms and systemic corticosteroid effects.
Clinical pharmacology
Beclometasone dipropionate is a glucocorticoid prodrug rapidly hydrolysed to beclometasone-17-monopropionate, which has substantially greater receptor activity. Formulations maximise local anti-inflammatory action while limiting systemic exposure, although systemic effects remain possible.
Formulation and product differences
- Qvar is an extrafine solution aerosol with different lung deposition and potency characteristics from many conventional beclometasone inhalers; switching requires product-specific review.
- The selected Soprobec product is a conventional pressurised inhalation solution for adult asthma maintenance; its device and spacer requirements differ from Qvar.
- Beconase is an aqueous intranasal suspension containing benzalkonium chloride, which can cause nasal irritation or swelling with prolonged use.
- Cream is often more suitable for moist or weeping lesions; ointment is greasier and often better suited to dry, thickened or scaly lesions.
- Clipper is a gastro-resistant prolonged-release tablet designed to release medicine later in the intestine and must be swallowed whole.

beclometasone dipropionate interactions
Systemic steroid effects can increase with some combinations.
Ritonavir, cobicistat and other strong CYP3A inhibitors
Systemic corticosteroid effects cannot be excluded; caution and appropriate monitoring are required.
Other corticosteroids
Combined exposure can increase adrenal suppression and other systemic effects.
