What matters clinically? Which features are concerning, which are reassuring? What is the differential? Which diagnoses should be actively excluded? A clinician presenting with acute headache needs to know which features distinguish tension-type from migraine from subarachnoid haemorrhage — not just what the guideline says about headache management in general.
Which guidance applies? Is there a NICE guideline? Does CKS have a primary care summary? Does the BNF address the prescribing question? Is there a local pathway that modifies the national recommendation? When multiple sources are relevant, which takes priority?

What should be explained to the patient? Shared decision-making requires translating clinical reasoning into language the patient understands. Reasoning support should help identify what needs to be communicated — including uncertainty, options, and red flags.
Why Healthcare Is Harder Than Ordinary Search
Local variation. The same question has different operational answers in different settings — different referral routes, formularies, service availability. Patient context.
