Pakistan is the second-largest source country for IMGs entering US residency. The pathway is well-established, with strong Pakistani physician communities across the United States providing mentorship, networking, and programme connections.
Degree Recognition
PMDC (Pakistan Medical and Dental Council)-registered MBBS degrees from recognised Pakistani medical colleges are listed in WDOMS. ECFMG recognises these degrees for US licensing eligibility. The 2024 accreditation pathway may affect graduates from some newer or less established institutions — verify your specific college's status on the ECFMG website before beginning preparation.
The Pathway
Complete MBBS and house job → USMLE Step 1 → Step 2 CK → OET → ECFMG certification → ERAS application → NRMP match. Most Pakistani graduates begin USMLE preparation during or immediately after completing house job.
Pakistan-Specific Challenges
Exam centre availability. Prometric centres in Pakistan have limited capacity and scheduling slots fill quickly. Many Pakistani IMGs schedule exams at UAE centres (Dubai, Abu Dhabi) — factor in travel costs, visa requirements, and accommodation. Currency and cost. USMLE fees are in US dollars. At current PKR exchange rates, the total investment ($15,000-25,000+) represents a very significant financial commitment requiring careful planning and often family support. Limited local prep infrastructure. Self-directed study with Q-banks is the dominant preparation model — formal USMLE coaching centres exist but vary in quality.
Specialty Matching Trends
Pakistani IMGs match most successfully in internal medicine, family medicine, and psychiatry. Building US clinical experience (observerships or externships) significantly strengthens applications — though obtaining these from Pakistan requires proactive networking through Pakistani physician associations in the US, programme contacts, and research collaborations.
Building a Competitive Application
Research publications (even case reports and systematic reviews) add measurable value. Strong letters of recommendation — ideally from US-based physicians. Target Step 2 CK scores of 240+ for IM, 250+ for competitive specialties. Pakistani IMG networks (APPNA chapters, regional medical graduate associations) provide mentorship and programme connections.
