India is one of the largest source countries for IMGs entering US residency. The pathway from MBBS to US practice is well-established but requires careful planning — typically 2-4 years from completion of internship to starting residency.
Degree Recognition
NMC (National Medical Commission, formerly MCI)-registered MBBS degrees from recognised Indian medical colleges are listed in the World Directory of Medical Schools. ECFMG recognises these degrees for US licensing eligibility. Verify your specific college's listing on the WDOMS website.
The Pathway
Complete MBBS and compulsory rotating internship → USMLE Step 1 (many start during internship) → Step 2 CK → OET (English proficiency) → ECFMG certification → ERAS application → NRMP match → US residency.
Timeline
Most Indian graduates take 2-4 years from MBBS completion to starting residency. Those who study full-time for USMLE can compress this to 18-24 months. Those working alongside preparation take longer.
Financial Planning
Exam fees, study materials, and ERAS application costs total $15,000-25,000+. Living expenses during the study period add substantially. Exchange rate fluctuations affect the real cost significantly. Plan financial resources before starting.
Coaching Centres
India has a large USMLE coaching industry. What adds value: structured study groups, experienced mentors, and accountability. What is often a waste: expensive lecture courses that replicate what self-study with a good Q-bank provides. A primary Q-bank (UWorld) + adaptive supplement (iatroX at $99/year) is more effective and far cheaper than most coaching programmes.
NEET PG Overlap
If you are considering both Indian PG and US residency simultaneously, the clinical content overlap is significant — especially in medicine, paediatrics, and OBG. The pharmacology and screening guidelines differ substantially. Do not assume NEET PG preparation alone prepares you for USMLE.
Visa Considerations
Most Indian IMGs enter residency on J-1 visas (requires 2-year home country return) or H-1B (no return requirement but requires Step 3 and programme sponsorship). Plan your visa strategy before you apply.
