Internal medicine is the most common specialty for IMGs — and the most accessible. But "IMG-friendly" varies significantly between programmes.
What Makes a Program IMG-Friendly
Historical IMG acceptance. Check programme websites, FREIDA Online, and residency community spreadsheets for the percentage of current residents who are IMGs. Programmes where 50%+ of residents are IMGs have a demonstrated track record. H-1B sponsorship. Not all programmes sponsor H-1B visas — this is a hard disqualifier if you need it. Verify before applying. Community vs university. Community programmes accept more IMGs overall. University programmes are more competitive but offer stronger academic environments and research opportunities.
Geographic Patterns
Midwest (Michigan, Ohio, Illinois, Indiana), Northeast (New York, New Jersey, Pennsylvania, Connecticut), and parts of the South (Florida, Texas) have the highest concentration of IMG-friendly IM programmes. West Coast programmes tend to be more competitive with lower IMG acceptance rates.
How to Research Programs
FREIDA Online (AMA's residency database — sortable by programme characteristics). Residency Explorer (AAMC). Programme websites (look at current resident photos/bios to count IMGs). Reddit r/residency and IMG-specific threads. Community-maintained IMG programme spreadsheets updated annually.
Application Strategy
IMGs typically apply to 100-200+ IM programmes. This is not excessive given the match statistics. Filter your list by: H-1B sponsorship (if needed), YOG requirements, Step 2 CK score cutoffs, geographic preference, and programme size.
Red Flags
ACGME citations for duty hour violations. High resident attrition rates. Very low board pass rates. Programmes where residents frequently transfer. This information is available on ACGME's public website and through residency community forums.
