The partnership model is in structural decline — fewer GPs choosing it, practice closures and mergers, super-partnerships emerging. But it still offers the highest earning potential and maximum autonomy. The question is whether the trade-offs are worth it for you.
Partnership
Average drawings typically higher than salaried salary — but variable by practice, list size, and region. Full autonomy over practice decisions — staffing, clinical focus, investment, premises. Financial risk: property liabilities, staff contracts, NHS contract obligations. Business management burden: HR, finance, CQC compliance, premises management. Declining uptake — but the model still supports the majority of UK general practice.
Due diligence before buying in: practice accounts (3+ years), list size trends, CQC rating and history, property lease terms and dilapidations liability, partner dynamics and agreement terms.
Salaried
BMA model salaried contract as starting point — negotiate from there. Predictable income (£70,000-100,000+ depending on sessions and experience). No business risk. Less autonomy — practice partners make strategic decisions. Limited career progression within the role itself. Good for lifestyle prioritisation, especially with young children or other commitments.
The Middle Ground
Salaried GP with additional sessions (locum days, portfolio activities). PCN roles alongside salaried sessions. Retainer schemes for reduced-hours practice. Many GPs combine salaried stability with locum flexibility.
Financial Comparison
Partnership drawings include business profits — but also require expense management, pension contributions, and tax planning. Salaried income is simpler but pensionable income calculations differ. Tax implications vary — partners are self-employed, salaried GPs are employees. Independent financial advice is worth the cost before making this decision.
Decision Framework
What stage of life are you in? What is your risk tolerance? Do you want to run a business or practise medicine? Is your priority income maximisation, work-life balance, autonomy, or security? There is no universally correct answer.
