What Form R Is

A self-declaration to your Responsible Officer confirming fitness to practise, health status, and any relevant concerns. It covers personal details, whole scope of practice (including any non-training work), significant events, complaints, health declarations, and probity declarations.

When to Submit

Before each ARCP panel — deadline set by your deanery (usually 2-4 weeks before panel). Check your deanery's specific timeline. Late submission can trigger Outcome 5 regardless of your portfolio quality.

How to Complete

Honestly and completely. Form R is a professional declaration — not a test you can fail by being honest. Declaring a health issue is not a failure; not declaring it is a probity concern. Reporting a complaint or significant event demonstrates professionalism; omitting it raises questions.
