Caribbean Medical School Graduates: USMLE Pass Rates, Match Data & Realistic Expectations

Caribbean medical schools occupy a specific position in the US medical education landscape — they provide an alternative pathway to US residency, but the data on outcomes varies dramatically between schools.

The Big 4 vs Others

St. George's University (SGU), Ross University, American University of the Caribbean (AUC), and Saba University are the most established Caribbean schools — with the highest USMLE pass rates and match rates among Caribbean graduates. Their outcomes, while lower than US MD schools, are significantly better than other Caribbean programmes.

Schools outside the Big 4 have highly variable outcomes. Some have USMLE Step 1 pass rates below 50% and match rates in the single digits. Research any school thoroughly before committing.

USMLE Pass Rates

NRMP and ECFMG data show Caribbean IMG Step 2 CK pass rates typically below US MD pass rates but above many other IMG cohorts. The variance between schools is enormous — Big 4 schools may approach 80-90% on first attempt while less established schools may be below 50%.

Match Data

Caribbean IMGs match most successfully in internal medicine, family medicine, psychiatry, and pathology. Competitive specialties (dermatology, orthopaedics, radiology) are extremely difficult for Caribbean graduates — the data is clear on this.

Attrition is the number Caribbean schools do not advertise. Some programmes lose 20-30%+ of students before graduation. The match rate is calculated from those who graduate and apply — not from those who enrol.

How to Maximise Your Chances

Target a Step 2 CK score of 240+. Complete US clinical rotations during your third and fourth year (Big 4 schools facilitate this). Build research experience. Secure strong US-based letters of recommendation. Apply broadly — Caribbean IMGs typically apply to 100+ programmes.

Financial Reality

Total cost of Caribbean medical education plus USMLE plus match application can exceed $250,000-400,000+. Factor this into your decision alongside realistic match probability for your target specialty.
