AI literacy is becoming a clinical skill — not a technology curiosity or an optional extra. The ability to use AI effectively, interrogate its outputs critically, maintain professional judgement in an AI-assisted workflow, and explain AI use to patients is increasingly relevant to pharmacy practice, exam performance, and patient safety.

A joint statement from UK statutory health and care regulators says learners are increasingly using generative AI to support learning and that, when used appropriately, AI can be positive. The statement also says education providers should help learners develop AI literacy — including the ability to identify biased, inaccurate, or misleading AI content, maintain confidentiality when using AI tools, and explain AI use clearly to patients or caregivers.

Why Pharmacists Need AI Literacy Now

AI tools are embedded in the clinical environment pharmacists work within — and the embedding is accelerating. Ambient scribes generate consultation notes that pharmacists review during medication reconciliation. Clinical search tools retrieve guidelines and medicines information that inform dispensing and prescribing decisions. Formulary tools suggest alternatives during medicines optimisation. Exam Q-banks use adaptive algorithms to target revision. CPD platforms use AI to structure reflections. General chatbots are used informally for clinical queries during busy shifts.

A pharmacist who cannot critically evaluate these tools faces specific risks. Accepting AI outputs without checking sources — "the AI said the dose is X" without verifying against the SmPC — may lead to dispensing errors if the AI is wrong. Not recognising hallucinated drug information — a plausible-sounding but fabricated drug interaction, an incorrect dose that is within a plausible range but not the licensed dose, a fictitious SmPC reference — may lead to clinical decisions based on false information. Not distinguishing between regulated SmPC data and generic internet content may lead to counselling based on US drug information rather than UK-licensed product details.

These are not theoretical risks. Real-world examples of AI hallucination in clinical contexts include fabricated drug names that sound plausible but do not exist (phonetically similar to real drugs), incorrect dose recommendations that are within a plausible therapeutic range but not aligned with the UK-licensed SmPC (e.g., a dose that is standard in the US but not the UK), interaction warnings that conflate two different drugs with similar names, and pregnancy safety assessments that contradict the current SmPC section 4.6 because the AI's training data predates a recent update.

AI literacy is not about being pro-AI or anti-AI. It is about being a competent professional in a clinical environment where AI tools are increasingly present — and that is now every pharmacy environment.

What AI-Literate Pharmacists Can Do

Interrogate sources. "Where did this information come from? Is it the SmPC, the BNF, a NICE guideline, or unspecified training data? Can I trace the claim to the original source and verify it?" This source-interrogation habit is the single most important AI literacy skill for pharmacists — because it converts an unverifiable AI response into a verifiable clinical reference. A pharmacist who checks the SmPC after every AI-generated counselling point is practising AI literacy. A pharmacist who accepts the AI's output without checking is not.

Spot hallucinations. "This drug interaction doesn't match what I remember from the BNF. Let me check the eMC interaction section before I act on this." "This drug name doesn't look right — is 'amoxiclavulanate' the standard UK name? The BNF says 'co-amoxiclav.'" Recognising when an AI response is confidently wrong — rather than accepting it because it sounds authoritative — prevents errors that source-naive users would miss. Hallucination detection is not a technical skill; it is a professional scepticism skill that pharmacists already apply to other information sources.

Recognise jurisdictional bias. "This AI may be trained primarily on US drug information. The US-licensed indication, dose, formulation, and monitoring requirements may differ from the UK SmPC. I need UK-specific data for this clinical decision." Understanding that AI training data has geographic bias — and that UK pharmacy practice requires UK-specific regulated information — is essential for safe dispensing and counselling.

Protect confidentiality. "I should not enter patient-identifiable information — name, date of birth, NHS number, address, specific clinical details — into an AI tool that is not governance-approved for processing patient data." Even if the AI could answer a question more precisely with patient details, the data protection and confidentiality implications make this inappropriate under UK GDPR and the GPhC Standards for Pharmacy Professionals.

Explain AI use to patients. Patients increasingly ask about AI in healthcare. A pharmacist should be able to explain: "This tool helps me check the medicine information quickly and accurately. The decision about your medication is mine, based on your individual medical history, your other medicines, and your specific situation. The tool helps me find the information faster — it doesn't make the decision for me."

Apply clinical judgement to AI outputs. "The AI suggests this standard dose. But this patient has an eGFR of 22 and is on warfarin. The standard dose may not be appropriate — I need to check SmPC section 4.2 for the renal dose adjustment and section 4.5 for the warfarin interaction before I confirm this is safe." This is where AI literacy and clinical competence converge: using AI as an input to professional reasoning, never as a substitute.
