ABFM certification is the family medicine board exam taken at the end of FM residency. For UK-trained IMGs especially, understanding how it differs from the MRCGP AKT helps target preparation efficiently.
Exam Format
Single-day exam, approximately 300 questions covering the full FM scope: paediatrics (well-child visits, developmental milestones, common childhood conditions), OB/GYN (prenatal care, labour management, contraception, common gynaecological conditions), adult medicine (cardiovascular, respiratory, endocrine, GI, MSK, dermatology, neurology), geriatrics, behavioural health (depression, anxiety, substance use, personality disorders), and preventive medicine (USPSTF screening, immunisations, counselling).
How ABFM Differs from MRCGP AKT
For UK-trained IMGs: ABFM covers broader procedural content (obstetric delivery, MSK procedures, dermatologic procedures). MRCGP AKT includes NHS-specific organisational questions; ABFM includes US healthcare system content (insurance, access, healthcare disparities). ABFM tests the US immunisation schedule (CDC) and USPSTF screening guidelines — both differ from UK equivalents. Patient-centred medical home concepts appear in ABFM but not MRCGP.
IMG-Specific Considerations
US preventive care guidelines (USPSTF — memorise grade A and B recommendations). Well-child visit schedules and developmental milestones per AAP (paediatric content is more heavily weighted in ABFM than most IMGs expect). US immunisation schedule (CDC — differs from UK and other national schedules). Insurance and access issues in clinical scenarios. Healthcare disparities and social determinants of health.
