A 68 year old man on Ramipril and Ibuprofen for osteoarthritis becomes acutely unwell with vomiting and diarrhoea. His creatinine has risen from 90 to 220 micromol per litre in 48 hours. He is hypotensive and oliguric. What is the most important immediate management step?
