A 52-year-old man attends his GP with episodic crampy abdominal pain, bloating and alternating diarrhoea and constipation for 9 months. Symptoms are worse with stress and improve after defaecation. There is no weight loss, rectal bleeding or nocturnal symptoms. Examination and basic blood tests are normal. What is the most likely diagnosis?
