A 72-year-old man is admitted with acute onset severe breathlessness at rest, orthopnoea and pink frothy sputum. He is hypertensive and tachycardic, with bilateral basal crackles and oxygen saturations of 88% on air. Chest X-ray shows pulmonary oedema.
