A 3-year-old boy presents with acute ear pain and fever 38.5 °C for 24 hours. He is otherwise well, fully immunised, with no otorrhoea. Otoscopy shows a bulging, erythematous right tympanic membrane without perforation. He has no risk factors for severe disease. What is the most appropriate initial management?
