A 26-year-old woman presents with lower abdominal pain and vaginal spotting at 7 weeks by last menstrual period. She has a positive pregnancy test. Transvaginal ultrasound does not show an intrauterine pregnancy, and there is a 2.5 cm adnexal mass with free fluid in the pelvis. She is haemodynamically stable. What is the most likely diagnosis?
