Concerns Regarding Long-Term Use of Ciclosporin: Long-term use of ciclosporin in atopic eczema is generally limited due to potential adverse effects and risks, including increased susceptibility to infections, which aligns with your patient's experience of minor infections, mouth ulcers, and cold sores. Ciclosporin is typically used for short courses and under specialist supervision because of its immunosuppressive effects and side effect profile. Recurrent infections, such as herpes simplex virus infections leading to cold sores, are known complications in atopic eczema and should prompt careful monitoring and possible reconsideration of therapy. Secondary infections constitute significant complications of atopic eczema and can be exacerbated by immunosuppressants like ciclosporin .

Consideration for Switching to Dupilumab: Dupilumab is an approved biologic therapy for moderate-to-severe atopic dermatitis in adolescents and adults who have had previous inadequate response to topical treatments. Unlike ciclosporin, dupilumab specifically targets interleukin-4 and interleukin-13 pathways involved in type 2 inflammation rather than broadly suppressing the immune system, which reduces the risk of infections and other systemic side effects ,,. Clinical trials demonstrate dupilumab's efficacy and safety in adolescents aged 12 years and older with moderate-to-severe atopic dermatitis, particularly when prior therapies were insufficient ,. Given the patient's ongoing minor side effects and the duration of ciclosporin treatment (one year), a switch to dupilumab could be considered, ideally in consultation with a dermatologist experienced in managing systemic and biologic treatments for eczema .

Recommendations: Review the patient for severity and impact of eczema and infection frequency. Consider referral or seek specialist advice for systemic treatment options when ciclosporin causes side effects or is unsuitable for long-term use. The decision to initiate dupilumab should be individualized, weighing the benefits against risks and discussed thoroughly with the patient, as it is administered by subcutaneous injection at intervals tailored to the patient's weight and disease severity ,,.
