The optimal clinic blood pressure target for a patient with type 2 diabetes mellitus is to reduce and maintain blood pressure below 140/90 mmHg if they are under 80 years of age.

For patients aged 80 and over with type 2 diabetes, the target is to maintain clinic blood pressure below 150/90 mmHg.

Home or ambulatory blood pressure monitoring targets are approximately 5 mmHg lower than clinic targets, so targets would be below 135/85 mmHg for those under 80 years and below 145/85 mmHg for those aged 80 or over.

Careful clinical judgement is advised in patients with frailty, multimorbidity, or signs of postural hypotension when setting or treating to blood pressure targets.
